Reopening During COVID-19

We are open with special policies in place to keep everyone safe.

By now you are probably familiar with the many altered procedures due to the pandemic. We will be asking the following of you:

When you come to your appointment, please wait in your car, or outside, or in the shelter of the garage until we phone you. Between five and ten minutes before your appointment, your therapist will call you to let you know that you may come up to the office. You will be asked about any possible symptoms of illness.
Please fill out the COVID-19 consent form below. Do not save the form and send it back to us, it will save blank. Please scan it, fax it or bring it to your next appointment. Thank you!

If you are unable to print the forms or would prefer to fill them out digitally please download the latest version of Adobe Reader, which you can download for free here.
This software will enable you to fill out the PDF, save it and email it to us at info@alexmyo.com.

Before your first appointment it is important to complete a medical history questionnaire. You will be asked to either come early and fill it out prior to your appointment or you may download the form, fill it out and bring it with you to your appointment.
Please also be sure to print, fill out, and sign the COVID Consent Form and bring it to your appointment. We will also have these forms in the office. Please find the forms here.
You may also fax your completed form to 703-549-2770.
If you are unable to print the forms or would prefer to fill them out digitally please download the latest version of Adobe Reader, which you can download for free here.
This software will enable you to fill out the PDF, save it and email it to us at info@alexmyo.com.

If you have any signs of illness please cancel your appointment. We have never charged for late cancellations when they occur because of illness. Please wear a mask when you enter the office. Your temperature will be taken with a nocontact thermometer. There will be hand sanitizer available as you enter, and also in the treatment room. If possible, you will be taken immediately to that room.

We are grateful for your patronage and for your patience and look forward to seeing you.
